Immune mediated hemolytic anemia (IMHA) in dogs is a severe disease with a high mortality rate. As human immunoglobulin (HIG) was reported to be beneficial for the treatment of IMHA in dogs we examined the influence of HIG on the course of the disease in our dogs with IMHA. Of 22 dogs with primary IMHA 9 dogs received in addition to routine immunosuppressive therapy HIG at a dose of 0.19 to 0.68 g/kg (median 0.35 g/kg), 13 dogs did not receive HIG (–HIG group). Both groups were similar in terms of age, weight, the presence of autoagglutination, spherocytosis, positive Coombs´ test, icterus and pigmenturia. The lowest hematocrit measured during the disease was significantly lower in the +HIG group compared to the –HIG group and dogs in the +HIG group received significantly more transfusions than those of the –HIG group. This is an indication for more severe disease signs of the +HIG group dogs. Although mortality during hospitalization and the time from hospital admission to release or death was not significantly different between the two groups, we interpret this similar course of the IMHA despite more severe signs of the +HIG group dogs as a potential positive effect of the HIG therapy.

Anwendung von humanem Immunglobulin bei Hunden mit primärer immunbedingter hämolytischer Anämie

Immunbedingte hämolytische Anämie (IHA) beim Hund ist eine schwere Erkrankung mit hoher Mortalität. Nachdem humanes Immunglobulin (HIG) als möglicherweise nützlich zur Therapie von IHA bei Hunden beschrieben wurde, haben wir in dieser retrospektiven Studie den Einfluss von HIG auf den Krankheitsverlauf einer primären IHA bei unseren Hunden untersucht. Von 22 Hunden mit IHA wurden 9 zusätzlich zur konventionellen, immunosuppressiven Therapie mit HIG (+HIG-Gruppe) in einer Dosis von 0.19–0.68 g/kg (Median 0.35 g/kg) behandelt; 13 Hunde blieben unbehandelt (–HIG-Gruppe). Die beiden Gruppen waren gleich zusammengesetzt bezüglich Alter, Gewicht, Vorhandensein von Autoagglutination, Sphärozytose, positivem Coombs-Test, Ikterus, und Pigmenturie. Während des Klinikaufenthaltes war in der +HIG-Gruppe der niedrigste Hämatokritwert signifikant tiefer als in der –HIG-Gruppe und Hunde in der +HIG-Gruppe erhielten signifikant mehr Bluttransfusionen als Hunde der –HIG-Gruppe, was auf einen schwerwiegenderen Erkrankungsverlauf hinweist. Obwohl die Mortalität während des Klinikaufenthaltes und die Zeit vom Klinikeintritt bis zur Entlassung oder bis zum Tod der Patienten nicht signifikant verschieden waren, interpretieren wir den ähnlichen Krankheitsverlauf bei Hunden mit unterschiedlich schweren Symptomen als Anzeichen einer positiven Wirkung der HIG-Verabreichung.

L´anémie hémolitique d´origine immunitaire (AHOI) chez le chien est une maladie grave avec une mortalité élevée. Après que l´immunoglobuline humaine (IGH) ait été reconnue comme potentiellement utile à la thérapie de l´AHOI chez les chiens, nous avons examiné au moyen de cette étude rétrospective l´influence de l´IGH sur le cours de la maladie chez nos chiens. Parmi 22 chiens atteints d´AHOI, 9 ont été traités avec une dose de 0.19–0.68 g/kg (median 0.35 g/kg) d´IGH (groupes +IGH) en plus d´une thérapie immunoréppressive conventionnelle. Treize chiens n´ont pas reçu d´IGH (–IGH). Les deux groupes étaient comparables en ce qui concerne l´âge, le poids, la présence d´auto-agglutination, de sphérocytose, d´un test de Coombs positif, d´un ictère et d´une pigmenturie. Au cours du séjour en clinique, l´hématocrite le plus bas du groupe +IGH était moins élevé que l´hématocrite correspondant chez le groupe IGH et les chiens du groupe +IGH ont reçu plus de transfusions de sang que les chiens du groupe –IGH ce qui démontre un cours de la maladie plus sévère. Quoique la mortalité pendant le séjour en clinique jusqu´à la décharge ou jusqu´au décès des patients n´était pas significativement différente, la similarité du cours de la maladie chez les chiens avec des symptômes de gravité différente peut-être interprétée comme manifestation d´un effet positif de l´administration d´IGH.

Nel cane l´anemia emolitica immunitaria (IHA) è una malattia grave con un alto tasso di mortalità. In considerazione del fatto che è stato descritto un effetto positivo dell´immunoglobulina umana (HIG) nella terapia dell´IHA, in uno studio retrospettivo è stato esaminato nei nostri cani l´effetto dell´HIG sul decorso della malattia in un´IHA primaria. Su 22 cani con IHA 9 sono stati trattati oltre che con la terapia convenzionale immunosuppressiva anche con HIG (gruppo +HIG) con una dose di 0,19–0,68 g/kg (valore mediano 0,35 g/kg), mentre 13 cani sono stati trattati con la terapia convenzionale (gruppo –HIG). I due gruppi corrispondevano per quanto concerneva l´età, il peso, la presenza di autoagglutinazione, di sl`erocitosi, di un test di Coombs positivo, di ittero e di pigmenturia. Durante la degenza in clinica l´ematocrito più basso del gruppo +HIG era significativamente minore di quello del gruppo –HIG ed i cani del gruppo +HIG hanno ricevuto un numero maggiore di trasfusioni di sangue dei cani del gruppo –HIG. Ciò indica un decorso della malattia più grave nel gruppo. Malgrado la mortalità durante la degenza in clinica ed il periodo tra il ricovero e la dimissione dalla clinica o la morte del paziente ì`ossero simili, interpretiamo un simile decorso della malattia in cani con sintomi di gravità diversa, come un segno indicante un effetto positivo dell´applicazione di HIG.
